| AN ACT in relation to criminal law.
|

| Be it enacted by the People of the State of Illinois, |
| 3 |
| represented in the General Assembly:
|
| 4 |
| Section 5. The Unified Code of Corrections is amended by |
| 5 |
| changing
Section 3-5-2 as follows:
|

| Sec. 3-5-2.
Institutional Record.
|

| The Department shall maintain records of the examination, |
| 9 |
| assignment,
transfer, discipline of committed persons and what |
| 10 |
| grievances, if any, are
made in each of its institutions, |
| 11 |
| facilities and programs. The record shall
contain the name of |
| 12 |
| the persons involved, the time, date, place and purpose
of the |
| 13 |
| procedure, the decision and basis of the decision
therefor, and
|
| 14 |
| any review of the
decision made.
