HOUSE RESOLUTION

 
2    WHEREAS, Community Health Partnership of Illinois (CHP) is 
3a Federally Qualified Health Center (FQHC) whose mission is to 
4build and deliver quality, culturally responsive health care 
5to the patients, caregivers, and communities it serves; and
 
6    WHEREAS, CHP empowers individuals to attain their best 
7health and well-being and has been serving the uninsured, the 
8underserved, and migrant and seasonal farmworkers and their 
9families in rural Illinois for more than 50 years; and
 
10    WHEREAS, CHP's network is comprised of six health and 
11dental clinics and multiple outreach sites located across 
12northern and central Illinois, and its health programs permit 
13employers with access to a qualified and thriving workforce, 
14ensuring Illinois has a strong and healthy economy; and
 
15    WHEREAS, CHP also provides students and their families 
16with much needed preventative health services at its physical 
17locations and from its mobile medical units with integrated 
18programs of care, linking and leading to the attainment of 
19better health outcomes; and
 
20    WHEREAS, CHP has been recognized by leaders of business, 
21industry, and rural communities for the services it provides

1while simultaneously bridging and closing the gap in health 
2care; and
 
3    WHEREAS, The CHP Kankakee Health Center will be 
4celebrating National Health Center Week and hosting an open 
5house on August 13, 2024; therefore, be it
 
6    RESOLVED, BY THE HOUSE OF REPRESENTATIVES OF THE ONE 
7HUNDRED THIRD GENERAL ASSEMBLY OF THE STATE OF ILLINOIS, that 
8we declare August 13, 2024 as Community Health Partnership of 
9Illinois Day in the State of Illinois; and be it further
 
10    RESOLVED, That a suitable copy of this resolution be 
11presented to Community Health Partnership of Illinois as a 
12symbol of our respect and esteem.
