Amends the Vital Records Act. Provides that the Department of Public Health shall issue a certificate of birth with the identity of the attending physician redacted upon request by: the person named on the certificate of birth, if the person is 18 years of age or older; a parent of the person named on the certificate of birth; the legal representative of the person named on the certificate of birth; or an attorney at law authorized in writing by the person named on the certificate of birth. Effective 120 days after becoming law.

2 | | Be it enacted by the People of the State of Illinois,
|
3 | | represented in the General Assembly:
|
4 | | Section 5. The Vital Records Act is amended by adding |
5 | | Section 17.1 as follows: |
6 | | (410 ILCS 535/17.1 new) |
7 | | Sec. 17.1. Redacted certificate of birth. The Department |
8 | | shall issue a certificate of birth with the identity of the |
9 | | attending physician redacted upon request by: |
10 | | (1) the person named on the certificate of birth, if |
11 | | the person is 18 years of age or older; |
12 | | (2) a parent of the person named on the certificate of |
13 | | birth; |
14 | | (3) the legal representative of the person named on the |
15 | | certificate of birth; or |
16 | | (4) an attorney at law authorized in writing by the |
17 | | person named on the certificate of birth. |
18 | | Section 99. Effective date. This Act takes effect 120 days |
19 | | after becoming law.
