2 | | contaminant leaked or suspected to have been leaked; |
3 | | (3) information as to whether the contaminant was |
4 | | leaked or suspected to have been leaked into the air, land, |
5 | | or water; |
6 | | (4) a brief description of the potential adverse health |
7 | | effects posed by the contaminant; |
8 | | (5) the name, business address, and phone number of |
9 | | persons at the Agency from whom additional information |
10 | | about the leak or suspected leak can be obtained; |
11 | | (6) the name, business address, and phone number of |
12 | | persons at the Department of Public Health from whom |
13 | | additional information about the health effects of the leak |
14 | | or suspected leak can be obtained; |
15 | | (7) the date, time of occurrence, and duration of the |
16 | | leak; and |
17 | | (8) if known, the wind direction during the leak. |
18 | | (b) The provisions of this Section apply only to an owner |
19 | | or operator of a sterilization source using one ton or more of |
20 | | ethylene oxide in a rolling 12-month period of sterilization or |
21 | | fumigation operations. This Section does not apply to: beehive |
22 | | fumigators; research or laboratory facilities, as defined in |
23 | | Section 112(c)(7) of Title III of the federal Clean Air Act; or |
24 | | sources such as hospitals, doctors' offices, clinics, or other |
25 | | facilities for which the primary purpose is to provide medical |
26 | | services to humans or animals. ".
