AMENDMENT TO HOUSE BILL 5654 




2    AMENDMENT NO. ______. Amend House Bill 5654 on page 1, line 
35, after "25.4", by inserting "and adding Section 25.6"; and


 
4on page 8, immediately below line 26, by inserting the 
5following:
 
6    "(410 ILCS 535/25.6 new)
7    Sec. 25.6. Fee waiver; persons who reside in a shelter for 
8domestic violence.
9    (a) The applicable fees under Section 17 of this Act for a 
10new certificate of birth and Section 25 of this Act for a 
11search of a birth record or a certified copy of a birth record 
12shall be waived for all requests by a person who resides in a 
13shelter for domestic violence. The State Registrar of Vital 
14Records shall establish standards and procedures consistent 
15with this Section for waiver of the applicable fees. A person 
16described under this Section must not be charged for

1verification under this Section. A person who knowingly or 
2purposefully falsifies this verification is subject to a 
3penalty of $100.
4    (b) A person who resides in a shelter for domestic violence 
5shall be provided no more than 4 birth records annually under 
6this Section.".
