6 | | Each individual residing in a community integrated living |
7 | | arrangement shall receive an annual assessment to screen that |
8 | | individual for any health issues or risks. Beginning July 1, |
9 | | 2019, each individual shall receive his or her annual client |
10 | | assessment via a web-based, electronic screening tool. The |
11 | | electronic screening tool shall replace the current |
12 | | paper-based assessment. A provider may make a request, along |
13 | | with justifications, to the Department to complete the |
14 | | assessment on paper. Subject to appropriation, the Department |
15 | | may contract with a third-party entity to create and implement |
16 | | the web-based, electronic screening tool. The Department shall |
17 | | make changes to its rules in the Illinois Administrative Code |
18 | | to incorporate a web-based, electronic assessment tool. |
19 | | "Case coordination" means a mechanism for linking and |
20 | | coordinating segments
of the service delivery system to ensure |
21 | | the most comprehensive program
for meeting an individual |
22 | | client's needs. It facilitates client movement
through an array |
23 | | of services so that at any given time, services received are

24 | | matched to the needs of the individual.
