Amends the Illinois Income Tax Act. Provides that the Department of Revenue shall print on its individual income tax forms a county designation. Effective immediately.

| 2 | | Be it enacted by the People of the State of Illinois,
|
| 3 | | represented in the General Assembly:
|
| 4 | | Section 5. The Illinois Income Tax Act is amended by |
| 5 | | changing Section 512 as follows:
|
| 6 | | (35 ILCS 5/512) (from Ch. 120, par. 5-512)
|
| 7 | | Sec. 512.
(a) All individual income tax return forms for |
| 8 | | tax years
ending December 31, 1986 through December 30, 1995 |
| 9 | | shall contain an appropriate
space in which the taxpayer must |
| 10 | | indicate either (i) the name and number of the
high school |
| 11 | | district in which they reside on the date such return is filed, |
| 12 | | or
(ii) the name and number of the unit school district in |
| 13 | | which they reside on
the date such return is filed. Failure of |
| 14 | | the taxpayer to insert such
information shall not invalidate |
| 15 | | the return.

| 16 | | (b) For all tax years ending December 31, 1995 and |
| 17 | | thereafter, the
Department shall provide the State Board of |
| 18 | | Education with information on
individual income tax receipts by |
| 19 | | school district from the data collected by
the Geographic |
| 20 | | Information System maintained by the Department. |
| 21 | | (c) Notwithstanding any other provision of this Act, for |
| 22 | | tax years beginning after December 31, 2017, the Department |
| 23 | | shall print on its individual income tax return forms a county |
