| 2 | | Be it enacted by the People of the State of Illinois,
|
| 3 | | represented in the General Assembly:

| 4 | | Section 5. The Nursing Home Care Act is amended by changing |
| 5 | | Section 2-106a as follows:
|
| 6 | | (210 ILCS 45/2-106a)
|
| 7 | | Sec. 2-106a. Resident identification wristlet. An |
| 8 | | identification wristlet may be employed for any resident upon a |
| 9 | | physician's order, which shall document the need for the |
| 10 | | identification wristlet in the resident's clinical record. A |
| 11 | | facility may require a resident residing in an Alzheimer's |
| 12 | | disease unit with a history of wandering to wear an |
| 13 | | identification wristlet, unless the resident's guardian or |
| 14 | | power of attorney directs that the wristlet be removed. All |
| 15 | | identification wristlets shall include, at a
minimum, No |
| 16 | | identification
wristlets shall be employed except as ordered by |
| 17 | | a physician who documents the
need for such mandatory |
| 18 | | identification in the resident's clinical record. When

| 19 | | identification bracelets are required, they must identify the |
| 20 | | resident's name,
and the name, telephone number, and address of |
| 21 | | the facility issuing the identification wristlet.
