    Sec. 2-8. Services provided by the Department under the Program shall be denied:
        (a) if the adult with a mental disability no longer meets the eligibility criteria,
        (b) if the adult with a mental disability submits false information in an application or
    
reapplication for participation in the Program, or
        (c) if the adult with a mental disability fails to request or access any services after
    
120 days. Prior to making the decision, if the adult with mental disabilities has failed to request or access any services within 90 days, the Department shall give written notice to the person who signed the application that participation in the Program will be denied if services are not requested or accessed within 30 days.
    Whenever services provided by the Department under the Program are denied for the reasons in paragraphs (a), (b), or (c) of this Section, the Department shall give written notice of the decision and the reasons for denial of services to the person who signed the application. Such notice shall contain information on requesting an appeal under Section 2-13.
