Sec. 14. A person shall be qualified for initial licensure as an optometrist
if that person has applied in writing in form and substance satisfactory to
the Department and who:
(1) (blank);
(2) has graduated, after January 1, 1994, from a program of optometry education approved |
| by the Department or has graduated, prior to January 1, 1994, and has met substantially equivalent criteria established by the Department;

(3) (blank); and
(4) has met all examination requirements including the passage of a nationally
|
| recognized examination authorized by the Department. Each applicant shall be tested on theoretical knowledge and clinical practice skills.
